We titrate medication, watch labs, and talk about the habits that actually move the numbers — with enough time that you are not left guessing between visits.

Adults with established chronic conditions, or newly diagnosed patients who need a medical home. Unstable or highly complex disease may be co-managed with specialty care.
Your visit
Intake maps diagnoses, meds, and recent labs. Visits are spaced to the condition — tighter after a change, looser when you are stable. We set targets you can see (blood pressure logs, A1c, TSH) and adjust.
You should leave knowing what "good" looks like before the next appointment.

How often will I come in?
As often as control requires. After a medication change, sooner; when stable, we stretch the interval and still keep lab dates.
Can you manage my meds if another doctor started them?
Yes, once we have the history and agree the regimen is safe. We do not blindly refill without a relationship.
